Remember that this a preference sheet. Include things that are most important and refrain from including things that you can control (temperature, music, etc). Topics can include anything from sharing your fears, to who you'd like to catch your baby, should it not be your HCP. Remember to bring this sheet with you to prenatal appointments so that your Doctor or Midwife and get to know your desires. This is also a wonderful way to open dialogue and be sure that your expectations are in line with the practice you have chosen.
